Alternate for "inaccuracy": inaccuracy, violation, falses reporting, fraudulent reporting
Alternate for "account": charge-off, inquiry, collection, bankruptcy, late payment, etc.
Example: delete this charge-off for violation because the monthly payment reports on a closed account in the amount of $15

For each disputed account, write a dispute reason using the AJERO formula:
Example:
[Action] this [account type] for inaccuracy [justifier] the [element] is [reason] and [outcome]
A. Delete this charge-off for inaccuracy because the date last active reports differently on each bureau (EX 1/1/2015, EQ 2/2/2015, TU 3/3/2015)
B. Remove this late payment for inaccuacy because I was not late on 3/3/2016 and update to paid on time never late A. Action (delete, update, remove, correct) B. Justifier (because)
C. Element (date last active, date last paid, balance, past due, etc.)
D. Reason (include facts taken directly from the credit report here; e.g. "EX reports [date], EQ reports [date], TU reports [date]")
E. Outcome (update to paid on time never late, change status to [new status]) 1

How to write the perfect letter
We can easily write a dispute letter by answering just a few simple questions:
Why are you writing this letter?
What do you want them to do?
Did you dispute before and receive a response? If not, it is recommended to include a sentence relating to their failure to respond within 30 days
Ask for a new credit report reflecting/showing/reporting the changes
Example:
I received a copy of my credit report online with IdentityIQ and noticed fraudulent reporting and inaccuracies that require your attention and deletion. I did send a dispute letter last month and no response was received within the 30-day required timing. Please remove all of the following accounts/inaccuracies immediately and send me a new credit report:
[add in the dispute reasons you just created]
Create your letter by answering the following questions below:
Why are you writing this letter? ________________________________________________________________What do you want them to do? ________________________________________________________________Did you dispute before and receive a response? Go through this checklist before sending out your dispute letter
A. Letter includes no more than 2 paragraphs before "disputed accounts"
B. Letter includes no more than 10 accounts (write an additional letter to include extra accounts)
C. Disputed accounts include no more than 1 dispute reason
D. Letter is void of "pursuant to", "section [xxx]", and other law-related items UNLESS attacking a violation
E. Letter includes name, date of birth, address, and SSN
F. Letter includes copies of ID, proof of address, and (optionally) proof of SSN

Secret #1: Removing Debt Buyer Collections Like Portfolio Recovery
An example dispute:
Delete collection for inaccuracy because it reports me as late on [date] and [date], even though this is a collection, which cannot report late payments
Secret #2: Removing 3rd-Party Collections Like Medical or the Gym
An example dispute:
A. This account is unknown and cannot be verified as accurate - section 623 requires a furnisher name and address be displayed conspicuously
B. I disputed this on [date] and the notice of dispute was required to report within 30 days. On [date], I pulled my credit report and the notice was not there. Delete this account for violation of section 623
Secret #3: Removing Late Payments
An example dispute:
A. I paid on time for [date] - please remove this late payment for inaccuracy and update to paid on time, never late
A (2). Remove this account for inaccuracy because I paid on time for [date]
B. I disputed this on [date] and the notice of dispute was required to report within 30 days. On [date], I pulled my credit report and the notice was not there. Delete this account for violation of section 623
Secret #4: Removing Charge-offs
An example dispute:
A. I paid on time for [date] - delete this account for false reporting
B. Account reports 3 different dates last paid, which means this account will be removed naturally on 3 different [months, years]. Delete this account
C. This account is closed but reports a monthly payment; delete
D. I disputed this on [date] and the notice of dispute was required to report within 30 days. On [date], I pulled my credit report and the notice was not there. Delete this account for violation of section 623
